Meet My Camper Form           

Download the Meet My Camper Form and send to Camp with your Camper the first night the Camper attends Camp Firefly.

Camper Pick-up Form          

Download the Camper Pick-up Form and bring nightly to pick-up your Camper(s). DO NOT SEND TO CAMP WITH YOUR CAMPER(S). The form must be given to the Nightly Carpool Pickup Coordinator.

Camper & Parent Evaluation Form          

 

Download the Camper & Parent Evaluation Form and send to Camp Friday night with you Camper(s). Campers that return the completed form on Friday.
